In line with new Data Regulations that are effective from 25th May 2018, Therapy Zone (Claire Edwards) states that any personal data that is collected via telephone or email from you, will be:  your full name and a brief insight into why you require an appointment. This information will be added to a Consultation form at your first appointment, which will contain: your full name, date of birth, address, GP and medical history.  The form will be signed by yourself and Claire in your presence, confirming you consent to treatment and that the information you have provided is accurate and up to date.  This information will be checked at any subsequent appointments to alter any changes.  If however, Claire wishes to seek a second opinion, medical advice or to write a report on your behalf, following discussion with you, your written consent will be obtained in order to write in a confidential manner, to a third party (usually medical/legal).  You will receive a copy of such correspondence if stated by you at the time of obtaining consent. 

Your personal data will be in hand-written format and stored securely at the treatment room premises.  Any computerized correspondence, namely reports/letters etc, are stored on an external hard drive in a secure filing cabinet at Claire's personal address.   All correspondence relating to you will be held for 10 years, as per professional body (F.H.T.) standards, after you have ended your treatments or are deceased.  Thereafter they will be destroyed by shredding means. 

Therapy Zone (Claire Edwards), will not send you any marketing correspondence via email or telephone, neither is Therapy Zone on any social media.  

Therapy Zone (Claire Edwards), will not sell nor distribute any of your personal data to another party unless you provide a written request for her to do so.
